I’m trying to find out about my aunt Dolores Amoh; her home phone has been out of service and I am trying to find out did she pass away, as I have no information on her only child Sidney Crowe. Could you please help me find out if she’s OK??

Hi, Clarence. Thanks for your question. In order to have taxes withheld from your Social Security benefit, you must start by printing, completing and submitting an IRS Voluntary Withholding Request Form (Form W-4V). On this form, you can choose to have 7, 10, 12, or 22 percent of your monthly benefit withheld. The Form W-4V (to withhold federal taxes from your Social Security benefits) can be returned to your local Social Security office by mail or in person. If you are receiving Social Security retirement or disability, you can create or log into your personal my Social Security account so that you can change Direct Deposit online yourself. If you are receiving Supplemental Security Income (SSI) benefits, please contact us by calling 1-800-772-1213 (TTY 1-800-325-0778), between 7:00 a.m. and 7:00 p.m., Monday through Friday. Also, your bank, credit union, or savings and loan can assist you with setting up your direct deposit for your new account. Please report to us a non-receipt of payment immediately. Hi Janice. We are sorry for your loss. You cannot report a death online. Normally, the funeral home reports the death to Social Security. But you can report your husband’s death by calling us at 1-800-772-1213 (TTY 1-800-325-0778), between 7:00 a.m. and 7:00 p.m., Monday through Friday.
