    I need to deliver the forms to my local social security office to initiate coverage under part B of medicare as I am retiring on Feb. 28. I was told the next available appointment is in March. So I will just have to walk in and waste a lot of time waiting. Is there a better solution?

      Hi, Marshall. You have the option to return the completed forms, Form CMS-L564 and Form CMS-40B to your local office in person or by mail. We hope this helps.

    1- I need my local office address and was not able to find it on this site.
    2- I have no idea why my Soc Sec amount is lower or what is being deducted — as I have several conflicting letters about MO paying my part B for 2020; a possible over error on part B for late 2018 & early 2019, and some repayment of Food Stamps ($15 x 6 mo)
    I was only able to get some of this corrected when I called the fraud line… As I tried repeatedly to inform SS & Local Food stamp office that I had a temp job for 6 mo (3 mo. at end of 2018 + 3 mo in early 2019)…
    Also– I never received my Federal or State refund for 2018 and now need a replacement form from 2018 SS.
    Or I need to report a fraudulent claim on my return

        I complained to NJ police that SSA employee of Hackensack NJ Ms. Me not threatened me that I have to lie about my 4-3-1982 car accident and 8 broken bones injuries because they are gAve my 1982 SSDI and disability to imposter and Ms. Me not said I have to lie and say my car accident and injuries happened now I refused to lie she said ok we will deny you again and Administrative law judge DENIED me disability under false hospital name and without DOCUMENTING my 8 broken bones. The police said go to Hackensack police and complain there because SSA is in Hackensack. I went to Hackensack police they did not give me a report. NJ SSA keeps on giving an imposter my 8 broken bones disability under my 1982 Elmhurst Hospital Medical Records of my 4-3-1982 car accident which imposter claimed it with Allstate and collected my SSDI they all ganged up against me but I am the real Ani Minassian who had a car accident on 4-3-1982 with MTA bus on Steinway St Astoria NY but SSA AND ALLSTATE AND MY EMPLOYER ALL GANGED UP AGAINST ME SUPPORTING THE IMPOSTER INCLUDING THE POLICE THEY THINK I AM THE IMPOSTER BUT I AM THE REAL INJURED. I AM THE REAL ANI MINASSIAN MIHRANIAN. THE POLICE HELPING THEM ABUSE ME LONGER BY NOT BELIEVING I AM THE REAL INJURED AND ELMHURST HOSPITALOF QUEENS IS CONNECTED WITH THESE GIANTS. THEY REFUSE TO GIVE ME MY MEDICAL RECORDS BUT GAVE IT TO THE IMPOSTER I FEEL LIKE I AM DEALING WITH THE MOBS SINCE THE DAY OF MY CAR ACCIDENT UNTIL NOW. EVEN WHEN I SUED AS PROSE THE 3 MILLION DOLLARS DEFAULT JUDGMENT WAS REWARDED TO THE IMPOSTER ANI MINASSIAN MIHRANIAN BUT I AM THE REAL ANI MINASSIAN MIHRANIAN CASE NO CV-2621/2018
        WHEN I COMPLAINED TO NY 108 POLICE THEY SAID WE CANT INVESTIGATE COURT THEFT BECAUSE THE COURT IS HIGHER THAN US. SOMEBODY THAT HAS POWER AND NOT CONNECTED WITH THESE CORRUPTED FEDERAL EMPLOYEES PLEASE CALL ME I AM THE INJURED IT HAS BEEN 38 YEARS OF ABUSE THE IMPOSTER COLLECTED AND COLLECTING ALL MY INJURIES MONEY.
