My SS card has been missing for years. I’m of the generation that used to carry our cards, although I kept it hidden in my wallet. I think I probably threw it away when I got a new wallet many years ago. My identity has never been compromised and I get a monthly report from my bank, so I don’t think it’s ever been used improperly. How do I get another one?

I cannot request a replacement card on line as I live in Minnesota. I have my certified birth certificate, my fathers birth certificate, no passport and nothing with my photo on it… what do i do??? can the form and the paperwork be sent in as i just had surgery and cannot drive for at least 4 months… help ….. Please see “Evidence of Identity” (page 2) for additional options here. You can submit the application with documents via mail. Keep in mind all submitted documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once you’ve submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.
