Where can I mail the SS 5 for a replacement card?

    • A.C.

Hi. First, realize that you may not need a replacement card. You will rarely need to show it. Knowing the Social Security number is what is important. To see if you’re eligible to apply for a replacement Social Security card online or to learn more on the process and what documents you will need to get a card, please visit our Social Security Number and Card page. Keep in mind all submitted documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once you’ve submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.

I need form SS5 to change my name. Your site is very unfriendly cannot get it to come up> Please send it to
    irishred475@aol.com
    Thank you

    • A.C.

Hi, Catherine. In order to change your name on your Social Security card, you must show us a document that proves your legal name change. If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records, you must show us an identity document in your old name (as shown in our records). It is the identity document (also known as your ID) that we will accept if it has expired; however, we still will need to see proof (legal document) of the name change. We offer the option for you to mail your completed application with original or certified documents to your local Social Security office. For more information about how and what to submit, visit our Frequently Asked Questions. We hope this helps.

I have a grand child he is 15. I need a replacement card. What document do you need. I have a birth certificate, and a student ID. Will that do?

    • A.C.

Thanks for your question. Generally, when applying for a replacement Social Security card for a child under the age of 18, we consider the parent(s) or step-parent with custody, as the proper applicant. Remember that knowing his number is what is important. You will rarely need to show his card, but if you need information on how to obtain a replacement Social Security card, visit our Frequently Asked Questions. You will need to present documents proving your identity, your grandson’s identity and citizenship status (if not already established), and evidence of your relationship to, or responsibility for, your grandchild. Also keep in mind the submitted documents must be originals. You can use birth certificates to prove your grandchild’s age or citizenship. But you cannot use it as proof of identity. We hope this helps.
