Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    My grandson’s parents have lost his social security card. He is 18. No ID of any kind. Does have his birth certificate. No license nor has he had a job, as of yet. What does he need to get a card? Our SS office here, said he needed a medical form of some kind. With his signature or a stamp. What would that be? Confused.

      Hi, Jane. We prefer a government issued document with your grandson’s picture, as proof of identity to replace his Social Security card. If he does not have one or he cannot get one within a reasonable amount of time, we may be able to use other documents such as:
      •Employee identification card;
      •School identification card;
      •Health insurance card; or
      •U.S. military identification card.
      Any documents he submits, must be an original or certified copy, current (not expired) and show his name, and other identifying information (date of birth or age).
      Also, realize that he may not need a replacement card. He will rarely need to show it. Knowing his Social Security number is what is important.       •Call us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7:00 a.m. to 7:00 p.m. At the voice prompt, say “helpdesk”; or
      •Contact or visit your local Social Security office. Your local office can assist with a replacement Social Security card. We hope this helps.

Knowing your Social Security number is what is important. You may only need to show it to an employer or service provider.     My husband and I have custody of our grandson and his last name has been legally changed to ours. When I go to our local SS office to get a replacement card for him reflecting his new name (bringing the court order and newly issued birth certificate), will I need to have my grandson with me or can I apply for his new card myself? He is sixteen.
