Our Online Benefits Application

Our online benefits application provides a high level of security and protection for the information you provide. Filing online gives you the freedom and convenience to file for various types of benefits including Retirement, Disability, Supplemental Security Income, and Medicare.

Follow these simple steps to begin:

  1. Review the information on our Retirement/Spouse’s, Disability, Medicare, or Supplemental Security Income Benefits page to make sure you meet the requirements to apply for benefits online and have the information you will need to do so.
  2. Create or sign in to your personal my Social Security account. 
  3. File your claim online.
  4. Check the status of your claim by signing in to your personal my Social Security account.

You should be able to complete and submit your application all at once. But if that’s not possible, you can return to your application and continue right where you left off!

After you’ve completed your application, we’ll also give you a confirmation number to check the status, and information on what to do next. If we are not able to process the request, we will provide specific information on how to contact us by phone or schedule an office appointment.

    I have completed my application for SS retirement benefits. How do I submit my application for SS retirement benefits?
    I do not see a submit button.

    I need to complete form CMS-1763 to waive Part B Medicare for now as I am full time employed with a company that has 20 or more employees. I was told to download this form, complete and return to Social Security Office. I have searched everywhere for this form.

    I thought I changed my 2020 income on the application I just submitted but as I clicked to submit I saw the amount is wrong. My application says $40,000 for 2020 income but it should be $55,182

      HI, David. If you are still unable to create an account or encounter a problem with your personal my Social Security account, you may:
      •Call us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7:00 a.m. to 7:00 p.m. At the voice prompt, say “helpdesk”; or
      •Contact your local Social Security office. We hope this helps.

    If the claimant is unable to fill out the forms is there a form I can fill out and be named as representative for her?

      Hi, Wilma. Generally speaking, if we decide the beneficiary is incapable of managing or directing the management of his or her benefits, we will select someone to serve as his or her representative payee. We usually look for family or friends to serve as representative payees. When friends or family are not able to serve as payees, we look for qualified organizations to be representative payees. If you are interested in applying to be a representative payee, you should read our publication, A Guide For Representative Payees, to understand a payee’s responsibilities. Hope this helps.
