    I am researching information for my mother, as I serve as her power of attorney. She has opted to change her current banking institution where she has her social security check deposited automatically. She needs a form to complete for said change. Where can I find said form for her to complete ? Please advise.

      Hi, Steven. Your mother can call us at 1-800-772-1213, Monday through Friday, between 7:00 a.m. and 7:00 p.m., for assistance. Generally, she will have a shorter wait if she calls later in the day. She can also contact her local Social Security office. Because of privacy and security concerns, Social Security cannot speak to you about your mother’s benefits. If your mother is not capable of managing and directing her Social Security benefits, someone should consider applying to be her representative payee. Even though you are power of attorney, you, or someone else, would still need to apply to serve as her payee. To learn more about becoming a representative payee, you can read our publication, “A Guide for Representative Payees” and visit the web page. You can also call us at 1-800-772-1213 (TTY 1-800-325-0778) to request an appointment. We hope this information helps.

The Federal Insurance and Self-Employment Contribution Acts (FICA and SECA) require the withholding of taxes from wages of employed people, and the net earnings of most self-employed people, for the Social Security and Medicare programs.
