    Hi,
    I have applied two times for replacement of my social security card. I send the form and copy of my ID. I was asked to mail my original Picture ID which I did. I got a letter asking me to provide more doc for name changing. I called the office and explained that I don’t want to change my name I just need a replacement. Still was not helped. They asked me to resend the Original Doc with form and re apply.
    Is there anyone who can help me with this issue.

    I have been trying to create a new account for a beneficiary, whom I am POA as well as rep payee over, and have had difficulty doing so because the individuals that were entrusted with her account, commited SSI fraud, which I have also had difficulty reporting. I need to be able to block them from accessing her records and also receiving the stimulus to the finance account they would not disclose to us, and labeled with only the facility name, not mentioning the benefiary. The misuse of the funds is extreme. please assist in how to resolve this matter. Her current benefits are depositing into another account succesfully, but there is one payment we don’t have access to along with the stimulus package

    My Date of Birth is wrong in the SSA system. How do I correct it?

Visit our Learn What Documents You Need for details on the documentation requirements. Under the heading, “Type of Card,” select “Corrected” for a list of the documents needed. You will need to provide evidence of your age (for example, a U.S. public birth certificate) and current documentation for evidence of identity.     I think you shouldn’t force older people to go green. Not everybody has good internet at home. Some cannot even afford it. Printers are absolute garbage and tend to fail on a regular basis requiring even more fixed income to be spent on that. Personal computers fail and get viruses and dump information stored there.
    Going green is damaging to older adults that are less computer literate. This is a policy for 20 years from now.
    Look at the comments. The people don’t understand and are now effectively locked out due to computer illiteracy.

    I recently moved out of state, the new state requires an SS card but I lost mine long ago. The online replacement site said it wouldn’t work for me, is it b/c my mailing address doesn’t match the address on my driver’s license? What can I do? The only photo ID I have is my license, and I can’t mail that it in w/ the form.

      Just called and asked the same question. The request has to be made by mail since the offices are not open due to COVID. Use form SS5 and mail it in. There’s a 2 week turnaround.

    Is there a cost to get a replaced social security card? I suspect my parents were scammed. The cost was $39…doesn’t seem right to me.

      No it’s not supposed to cost just showing proof of what they need but if you scroll down when filling it out on here they are charging to help prep it or something like that.

          I’m trying to get a replacement card for my 15 year old daughter. I called my local office and they said to complete the application online and mail in a copy of a medical record signed by her doctors office. However the replacement card application online says you must be 18 to use it. Please advise.

A proper applicant for a Social Security card is a person who may file an application for a card on behalf of himself/herself or on behalf of someone unable to file on his or her own behalf. For a child under age 18 living at home with both of his or her parents, the child’s parents could each be considered proper applicants for a card on behalf of the child.
