We’re pleased to announce that we’ve added Connecticut, Ohio, and Utah to the growing list of states where you can go online to replace your Social Security card. In many cases, you may not need a replacement card. Most of the time, simply knowing your Social Security number is enough.
If you do need to replace your lost or misplaced Social Security card, our online application makes getting a replacement card easier than ever. Online card replacement is available if you live in the District of Columbia or one of the 43 states that can verify state ID information for us. If you’re only requesting a replacement card and you’re making no changes, you may be able to use our free online service.
All you need to do is create a personal my Social Security account and meet certain requirements. Opening a personal my Social Security account is easy, convenient, and secure. We protect your information by using strict identity verification and security features. Once you have a personal account, simply follow the instructions to request a replacement Social Security card.
You can apply for a replacement card online, if you:
- Are a U. S. citizen age 18 or older with a U.S. mailing address (this includes APO, FPO, and DPO addresses);
- Are not requesting any changes to your card (including a name change); and
- Have a valid driver’s license or state-issued identification card.
Now is a great time to go online because Social Security offices are currently closed to the public due to the COVID-19 pandemic. If you need a replacement card, please visit our website to find out if you can take advantage of this convenient online replacement service.

Hello,
I got married recently and need to change my name on my social security card. I have called my local Social Security Office twice about this matter. The two ladies who helped me were very nice. They said I have to get the ss5 form online, print it, fill it out, and mail it to their office with my marriage certificate. The problem is that when I search for the form online, I can’t seem to find it. The search results are not what I need. Can someone please email me the form (not the website). I would really appreciate it. I can’t officially change my name at the DNV, health insurance, or my job etc. without the new social security card (with new name).
Thank you,
Aracely

Aracely, thank you for reading our blog and for your question. Many of our forms can be downloaded from http://www.ssa.gov/forms, including the SS-5, Application for a Social Security Card. Check out our Social Security Number and Card web page for instructions on how to get a new, replacement, or corrected Social Security card. Any documents you mail to us must be originals or copies certified by the record holder. We will return them to you. If you cannot download the SS-5, please use the automated services at 1-800-772-1213 or call your local office and ask the representative to mail you the form. You’ll find the phone number and mailing address with our Social Security Office Locator. We hope this is helpful.

How can you get a replacement card for a minor in Texas?
Cynthia
My son’s social security card is lost we need to request a replacement. He is 18 and we tried to set up an account on line. Because we cannot verify financial information he is not able to do this online. Is there any other way to request online without financial information? He does not have a credit card, does not receive social security benefits and does not have a w-2 or tax documents. Help is greatly appreciated as we need this asap. Thanks!
V.V.
Hi Cynthia, thanks for using our blog. Your son should visit our Social Security Number and Card page to learn how to get a replacement Social Security card. If he just needs proof of his number, he can call his local office and request that we mail him an SSN printout instead of a replacement Social Security card. Unfortunately, we are unable to process replacement Social Security card requests by phone at this time.

To whom it may concern, I lost my social security card again (absolutely not the 1st time). I’m trying to find work which is hard enough with covid & a history of alcoholism, homelessness & drug addiction. I had a few minimum wage offers but no one will hire me without my ss card or proof of it. My name is Ramon Ramos born 12/31/62 in the Bronx ny. If you can replace my card or mail me a letter it would really help a man in need. Thank you

It states above that online request for lost social security card is now accepted in CT. However, MySSA says it is not. Because of COVID, my local office is closed, and they tell me to fill out the required form and submit it with my passport or other accepted document. I am reluctant to use the mail. When will CT residents be able to use the online service?? Thanks.

How do I get a replacement SS card for a minor child in California? My child is getting a driver’s license and the DMV requires to see the original card. Since I am not her, I assume I can’t order one for her online. Where is the link for the form if you don’t qualify to order one online?
