Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    I live in Hawaii and have lost my original Social Security Card. How do I replace it?

    “My Social Security Account” will not let me complete the set up online. In fact, I am now blocked for 24 hours. I am computer saavy; however, this online process is not user friendly.

    How do I get form SS5 mailed to me?

      Hi, Darla. First, realize that you may not need a replacement card. You will rarely need to show it. Knowing the Social Security number is what is important. If you are still unable to create an account or encounter a problem with your personal my Social Security account, you may:
      •Call us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7:00 a.m. to 7:00 p.m. At the voice prompt, say “helpdesk”; or
      •Contact your local Social Security office. We hope this helps.

    Helping an elder brother-in-law get a replacement SS Card. He does not have internet and only a limited phone (usually does not have minutes to use it!!) Wondering if there is a paper form to use to get the card. Thanks.

      Hi, James. First, realize that your brother-in-law may not need a replacement card. He will rarely need to show it. Knowing the Social Security number is what is important. To learn more on the process and what documents he will need to get a card please visit our Social Security Number and Card page. Keep in mind all submitted documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once he has submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.

    I need a replacement LETTER for the letter that I received in November, 2019. Last year, this letter was dated November 21,2018. Under the date was BNC# : 18IB469F58413-A.The letter indicates, in part, the new amount of monthly deduction for the Medicare Part B premium.

    This site says I can get a replacement card online using the my Soc Sec account. I have an account which says the following:
    We’re sorry. We cannot process your request.
    You cannot request a replacement card online.
    Please contact your local office if you need a replacement card.
