My father is 88 years old and living in Florida for the winter. He had an injury which put it in the hospital. The hospital workers told him he had to get a new SS Card because his current card is too hard to read.
My father has tasked me with this chore. I am his power of attorney but have no photo id (in Ohio) or birth certificate for him. How can I proceed? Local SS offices have no phone access and are in a not-easily accessible office in Cleveland’s city center.

Hi, Diane. Thank you for your question. We prefer a government issued document with your father’s picture, as proof of identity to replace his Social Security card. If he does not have one or he cannot get one within a reasonable amount of time, we may be able to help him. To see if he’s eligible to apply for a replacement Social Security card online or to learn more on the process and what documents he will need to get a card, please visit here. If your father is not capable of managing and directing his Social Security benefits, someone should consider applying to be her representative payee. Even if you are power of attorney, you, or someone else, would still need to apply to serve as his payee. To learn more about becoming a representative payee, please visit our Representative Payee webpage. We hope this helps.

Setting up an online account was a breeze, but am told on line that I cannot request a replacement social security card on line, reason is I’ve lost mine. I meet all the criteria listed, so why can I not request it on line? Tried calling “help desk” only to be told by voice driven system that no on was available. Very frustrating.
