To see if you’re eligible to apply for a replacement Social Security card online or to learn more on the process and what documents you will need to get a card please visit our Social Security Number and Card page. Keep in mind all submitted documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once you’ve submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.

My wife has dementia. She has previously executed a power of attorney designating myself, my daughter and my son health-care co-agents. I, her husband, am her primary care person. I carry her SS card and other identification with me always. Can I also get a duplicate SS card for my daughter, who is very actively involved in her care.

Hi Richard. If your wife is not capable of managing and directing her own Social Security benefits, either you or one of your children should consider applying to be her representative payee. Even though you and your children have power of attorney, someone would still need to apply to serve as her representative payee for Social Security purposes. To learn more about becoming a representative payee, you can read our publication titled, “A Guide for Representative Payees.” You may also call us at 1-800-772-1213 (TTY 1-800-325-0778) Monday through Friday, between 7:00 a.m. and 7:00 p.m. to request an appointment at the local Social Security office to become her representative payee. Generally, when applying for a replacement Social Security card for a child under the age of 18, we consider the parent(s) or step-parent with custody, as the proper applicant. Remember that knowing his number is what is important. You will rarely need to show his card, but if you need information on how to obtain a replacement Social Security card, visit our Frequently Asked Questions. You will need to present documents proving your identity, your son’s identity and citizenship status (if not already established), and evidence of your relationship to, or responsibility for, your child. Also keep in mind the submitted documents must be originals. You can use birth certificates to prove your child’s age or citizenship. But you cannot use it as proof of identity. Hi, Julie Anne. In order to change your name on your Social Security card, you must show us a document that proves your legal name change. If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records, you must show us an identity document in your “old” name (as shown in our records) and another document showing your “new” name.
