Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    Medicare Card has my name as:

    My middle name is Sharon.
    My SS card has my name as:

    My Driver’s licens has my name with my maiden name:

    My Passport has my name with my maiden name:

    HOW SHOULD I REALLY HAVE MY SS card?

    I received an email saying my state of Kansas can now receive an online social security card replacement card. I tried online in my accoand it says I’m not eligible

      Hi, Gary. We are sorry to hear that. First, realize that you may not need a replacement card. You will rarely need to show it. Knowing the Social Security number is what is important. If you encounter a problem with your personal my Social Security account, you may:
      •Call us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7:00 a.m. to 7:00 p.m. At the voice prompt, say “helpdesk”; or
      •Contact your local Social Security office. We hope this helps.

    I live in Kansas and today attempted to get a replacement social security card and it said I couldn’t request one on line. Evidently the email I received is in error?

Keep in mind all submitted documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once you’ve submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.

    My husband has misplaced his ss card. I found and printed the form to fill out to get anew one. Can I take it to the SS Office for him? He would have to take off work to do it Thank You, Elizabeth Watson

Yes, you can bring your husband’s completed and signed application with his (original or certified) documents to the local Social Security office or the nearest Card Center in your area.
