Hello Brenda. Generally, to apply for a replacement Social Security card, individuals age 18 and older who are physically and mentally capable of reading and completing the application, must sign it and provide at least one document to prove their identity.
If you complete and sign the application on behalf of someone else, you must provide evidence that shows the individual is unable to apply on his or her own behalf. Evidence from a third party (e.g., a physician) must specifically address capability related to applying for an SSN card. Also, you must provide evidence that shows your authority (relationship) to sign the application, as well as documents to prove your identity and the identity of the person for whom you are filing the application.
We prefer a government issued document with your picture, as proof of identity. Any document you submit, must be an original or certified copy, current (not expired) and show your name, and other identifying information (date of birth or age).
To learn more on the process and what documents you will need to get a card, please visit our “Social Security Number and Card” web page. Some individuals may be eligible to apply for a replacement Social Security card online.
In most cases, you can take or mail the completed application with the supporting documents to the nearest Social Security office or Card Center that serves your area. Any documents you mail to us will be returned to you.
Remember, you don’t always need a replacement card. You will rarely need to show it. Knowing your Social Security number is what is important.

Hello Hugh. You will need to contact the nearest Social Security office in your area. You can use the Canadian Service Area Directory to determine which Resident Office is near you. For security reasons, we do not release Social Security numbers by phone.
We recommend that individuals living outside the United States contact the nearest Federal Benefits Unit or U.S. Embassy in their area. Also, our Office of International Operations home page provides more information to assist our customers living abroad.

Hello Sheryl. You will need to apply for your child’s replacement Social Security card. Take the completed application and documents to your local Social Security office or the nearest Social Security Card Center in your area.
