Sign up for Medicare Part B Online, by Fax or Mail

For many people, signing up for Medicare Part B doesn’t require you to leave the comfort of home.  Please visit our Medicare Part B webpage if:

  1. You are already enrolled in Medicare Part A.
  2. You would like to enroll in Part B during the Special Enrollment Period.

You can complete form CMS-40B (Application for Enrollment in Medicare – Part B [Medical Insurance]) and CMS-L564 (Request for Employment Information) online.

You can also fax the CMS-40B and CMS-L564 to 1-833-914-2016; or return forms by mail to your local Social Security office.  Please contact Social Security at 1-800-772-1213 (TTY 1-800-325-0778) if you have any questions.  

 Note:  When completing the forms:

  • State, “I want Part B coverage to begin (MM/YY)” in the remarks section of the CMS-40B form or online application.
  • If your employer is unable to complete Section B, please complete that portion as best you can on behalf of your employer without your employer’s signature.
  • Submit one of the following types of secondary evidence by uploading it from a saved document on your computer:
    • Income tax returns that show health insurance premiums paid.
    • W-2s reflecting pre-tax medical contributions.
    • Pay stubs that reflect health insurance premium deductions.
    • Health insurance cards with a policy effective date.
    • Explanations of benefits paid by the GHP or LGHP.
    • Statements or receipts that reflect payment of health insurance premiums.

    Hi, I sent my Request of Registration for Medicare Part B via Fax and Mail on 02/01/2021 to SSA Office Pembroke Pines Florida, but I did not have received confirmation they were received. I was under my husband insurance until 12/31/2020. Now I am uninsured. Please help me.

    Iam trying to enroll in Medicare Part B. I have been retired since jan. 31, 2021.

    I submitted an Online Application for Medicare Part B with SEP. I phoned the 1-800 number to confirm if SSA have received my application and how soon it can be processed. Rep says it is not showing in my file even though I printed the confirmation that I am done signing. I submitted the Online Application 3 times and it still does not show in my file. Is there a problem?

    I am 65 recently retired from a university position in South Korea. I have no problem with my Medicare A or B.

    My wife however is 70. She signed up to receive Social Security when she turned 65 and was automatically enrolled in Part A. She did not sign up for Part B because as my wife she was covered under the National Health insurance as long as I was working, which means last August.

    We are trying to enroll her in Part B. I have filled out the Form 40-B and included statements that fulfill the last option “Statements or receipts that reflect payment of health insurance premiums” that date from the year she turned 65, 2015, until August, 2020.

    We have submitted that application but the representative at our local office is insisting that the form must be signed by someone at the university in Korea certifying that my wife was covered by a group health plan; only the ‘group health plan’ is the Korean National Health Service.

    This is difficult to do, since they are in Korea, and it’s not a group health plan. Do you have a suggestion on how to resolve this?

        What time zone is that 800 number?

          Hi Eric, thanks for the question. The National 800 Number Network (1-800-772-1213) provides agent service five days a week from 8AM to 7PM in the caller’s time zone. 24-hour automated services are available 365 per days a year. Normally, call volumes are at their highest levels during the first week of the month and at check delivery times. We hope this helps!

    I completed the Medicare Part B application online on 2/17/21 and realized that I didn’t upload the property document. How do I upload the proper document to my application and to avoid delays in processing?

You should discuss your situation with a Social Security representative who can tell you the best way to get the proper document to the local office.     Hi,

    I have faxed a Medicare Enrollment a week ago, but there was no indication of receipt from your fax. I tried to search in Medicare online to see if any status is posted of my application, but nothing is posted. I had tried calling twice but no one picked up. This is the office I faxed to and tried to call.
