Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    I recently sent in an ss5 form with my driver’s license. I forgot to include my divorce Decree showing that my name was needed to be changed. I called and was told to send a copy of it separately with my name and ss# on it. I received my driver’s license back. Today I got a new social security card but it has my previous name on it. I cannot get a new driver’s license or change my name on my bank account until this is done. Could you please email me the correct ss5 form?

You can call us at 1-800-772-1213 for assistance or you can contact your local Social Security office. Please look for the general inquiry telephone number at the Social Security Office Locator.     My original Social Security card, issued decades ago, has my correct name:
    “Laszlo Bela Tamas”

    Now that I have retired (2019), I find out that my Medicare card – and in multiple places on-line – have my name mis-spelled (one single letter is incorrect):

    “Laselo B Tamas”

    It seems very important that my name be spelled correctly on all Federal Government documents.
