Hi, Frank. For your security, we do not have access to private information in this venue. We ask that members in our Blog community work with our offices with specific questions. You can call us at 1-800-772-1213, Monday through Friday, between 7:00 a.m. and 7:00 p.m., for assistance. Generally, you will have a shorter wait if you call later in the day. You can also contact your local Social Security office. We hope this helps.

how do I get a copy of my child’s social security card

    • L.A.

Thanks for your question. Generally, when applying for a replacement Social Security card for a child under the age of 18, we consider the parent(s) or step-parent with custody, as the proper applicant. Remember that knowing their number is what is important. You will rarely need to show their card, but if you need information on how to obtain a replacement Social Security card, visit our Frequently Asked Questions. You will need to present documents proving your identity, your child’s identity and citizenship status (if not already established), and evidence of your relationship to, or responsibility for, your child. Also keep in mind the submitted documents must be originals. You can use birth certificates to prove your child’s age or citizenship. But you cannot use it as proof of identity. We hope this helps.

Hi Donna. Please realize you may not need a replacement card. You will rarely need to show it. Knowing your Social Security number is what is important. However, if you must get a replacement card, and you live in one of the States shown on the map on this webpage, we have good news! You can use your personal my Social Security account to get a replacement Social Security card. We hope this helps.

I would like to get a replacement SSN card for my daughter Patricia Eskenazi. She is not yet 18 so I cannot do this online. I do have her SSN number but I don’t have the actual card. I cannot get through to an actual person on the help line. It is VERY FRUSTRATING! I simply want to know:
    1) where can I find the form SS5 online? Cannot find a link that doesn’t take me to the MySSN website
    2) can I go in person to the SSN office nearest me to get a replacement card? And if so, do I need to bring my daughter? I have her passport, copy of her SSN card, and her birth certificate.

Only a few states offer it. You don’t provide a direct link to the application doc. This is a nearly useless page.
