Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    Need replacement SS and Medicare cards. How do I request them?

      Hi James, thank you for your question. First, realize you may not need a replacement Social Security card. You will rarely need to show it. Knowing your Social Security number is what is important. However, if you must get a replacement card, you may be able to apply online. If you do not qualify to apply online, then you can apply by mail or visit your local office with required documentation. All documents must be either originals or certified copies.       Hi, Heather. Generally, when applying for a replacement Social Security card for a child under the age of 18, we will consider the parent(s), as the proper applicant. If the child (under age 18) is physically and mentally capable of completing an in-person interview or paper application for a Social Security card, the child may file on his or her own behalf.       Hi, Erica. First realize that you may not need a replacement card. You will rarely need to show it. Knowing the Social Security number is what is important. To see if you’re eligible to apply for a replacement Social Security card online or to learn more on the process and what documents you will need to get a card please visit our Social Security Number and Card web page. Keep in mind all mailed documents must be either originals or copies certified by the issuing agency and must (unless otherwise indicated) have been issued within the last two years. We cannot accept photocopies or notarized copies of documents. Once you’ve submitted all the needed documents, it can take up to 10 business days from the date we received the application to mail a replacement card. We hope this helps.

    I went to the DMV here in Delaware and they wouldn’t accept my Social Security documentation addressed to my new address here, to change my New Jersey Drivers Licence to a Delaware one because of my passport stating my name is Vincent Gerard White Sr. and my Social Security doesn’t have the Sr. after my last name.How do I obtain a new card with the Sr. Thank You in advance for your assistance with my concern.

      Hi, Vincent. You cannot apply for a replacement Social Security card online, when correcting your name. In order to change your name on your Social Security card, you must show us a document that proves your legal name change. If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records, you must show us an identity document in your old name (as shown in our records), and another identity document with your new name. We prefer a government issued document with your picture, as proof of identity. Any document you submit, must be an original or certified copy, current (not expired) and show your name, and other identifying information (date of birth or age). We offer the option to mail your completed application with original or certified documents to your local Social Security office or nearest Card Center in your area. We hope this helps!
