Need a Replacement Social Security Card?

You can now apply for a replacement Social Security card online using your personal my Social Security account. If you meet all requirements and live in some states or the District of Columbia, you can start using this service today. Go to www.socialsecurity.gov/myaccount for more information.

    I have filled out a FORM SS-5 LONG WITH BIRTH CERTIFICATE FOR A REPLACEMENT CARD. ON DECEMBER 4, I RECEIVED A LETTER REQUESTING DOCUMENITION BUT THE LETTER DOES NOT SAY WHAT DOCUMENTS ARE REQUESTED. INSTEAD THE S.S. OFFICE SENT ME ANOTHER SS-5 FORM TO FILL OUT.
    DOES NOT THESE PEOPLE THAT WORK FOR THE SS OFFICE HAVE ANY COMMON SENSE THAT WHEN THEY GET THE FILLED OUT FORM, NOT TO MAIL ANOTHER ONE TO BE FILLED OUT AGAIN.

      Hello Michael. Generally, we only use a birth certificate when we -first- establish your Social Security record.
      We prefer a government issued document with your picture, as proof of identity to replace your Social Security card. If you do not have one or you cannot get one within a reasonable amount of time, we may be able to use other documents such as:
      •Employee identification card;
      •School identification card;
      •Health insurance card; or
      •U.S. military identification card.
      Any document you submit, must be an original or certified copy, current (not expired) and show your name, and other identifying information (date of birth or age).
      Also, realize that you may not need a replacement card. You will rarely need to show it. Knowing your Social Security number is what is important.

    I want to apply for another ssn card for my son i lost it and i also forgot my son ssn number. I walk in yesterday to your office here on island and they told me to apply it online.

    I did not receive a new social security card.
    I changed residency from previously living in CO to living in TX in April 2018. Then traveling for the Summer and Fall I have lost mail due to Forwarding and Holding at USPS. There is mail that I did not receive (like bills) thus I know my mail has been lost in the system. I didn’t think about my new SSI card until I need it to sign up for a new Medgap policy this coming year and realize that I did not receive it. Can you help me recover my new card?

    I have 2 teenage children both under the age of 18 that I need replacement SSN cards for. Do they have to be present or can I bring in all their information and do it myself?

      Parents and legal guardians may complete and sign applications for replacement Social Security cards for children under age 18. However, you must provide evidence that shows your authority (relationship) to sign the application, as well as documents to prove your identity and the identity of the person for whom you are filing the application.
      We prefer a government issued document with your picture, as proof of identity. Any document you submit, must be an original or certified copy, current (not expired) and show your name, and other identifying information (date of birth or age).

      When finish, you can take or mail the completed application with the supporting documents to the nearest Social Security office or Card Center that serves your area. Any documents you mail to us will be returned to you.
      Remember, you don’t always need a replacement card. You will rarely need to show it. Knowing your Social Security number is what is important!

    Why drag me down to asking for VISA number & kicking me out of request of ID just to get a replacement SS card for MD drivers license? Why I have to open a SS account with many curves & side hick-ups just to get a replacement card. Orig card was eaten mostly by an animal in woods where wallet was lost back in 1970s!

    I need to replace my son’s social security card since it is missing. He can’t do it through a my social security account because he isn’t 18 years old. There should be a way to file for a replacement card online for minors who can’t have an account OR a way to set up a my social security account for a minor.

      If you are locked out, you can call us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7 a.m. to 7 p.m. At the voice prompt, say “helpdesk”. We hope this information helps.
