Our Online Benefits Application

Our online benefits application provides a high level of security and protection for the information you provide. Filing online gives you the freedom and convenience to file for various types of benefits including Retirement, Disability, Supplemental Security Income, and Medicare.

Follow these simple steps to begin:

  1. Review the information on our Retirement/Spouse’s, Disability, Medicare, or Supplemental Security Income Benefits page to make sure you meet the requirements to apply for benefits online and have the information you will need to do so.
  2. Create or sign in to your personal my Social Security account. 
  3. File your claim online.
  4. Check the status of your claim by signing in to your personal my Social Security account.

You should be able to complete and submit your application all at once. But if that’s not possible, you can return to your application and continue right where you left off!

After you’ve completed your application, we’ll also give you a confirmation number to check the status, and information on what to do next. If we are not able to process the request, we will provide specific information on how to contact us by phone or schedule an office appointment.

    I SHAMEKA BROWN WADE, IS REQUEST ING 2 APPLICATION FORM FOR MY 2 SONS DUE TO THE FACTED My WALLET WAS STOLEN N I CANT FIND THEIR SOCIAL SECURITY CARDS..In the only thing I have is their birth certificates, immunization shot records.IM WONDERING WHAT ELSE DO I MAY POSSIBLY NEED.DUE TO THE FACT IM APPROVED FOR OKLAHOMA HOUSING FINANCE AGENCY IN THEY INFORM ME I REALLY NEED MY SONS SOCIAL SECURITY CARDS I NEVER LOOSE IMPORTANT DOCUMENTS LIKE THIS..I DNT have transportation.If it’s possible can u please send me the application so their social security card can get mail before my initial BRIEFING.THANKS I WILL MOST DEFINITELY APPRECIATE IT..

      Hi Shameka. If you are a “guardian,” our guess that your sons are under the age of 18. If the children are under 18 years old, then you can only get a replacement Social Security number card in person or by mail.     I completed my S.S. retirement application online today and haven’t received any type notification from the SS office. They have my email and other contact info!

      Hi Sue. If you wish to terminate your enrollment, a signed request for termination and a personal interview is required. You will have to go to your local Social Security office, and we will help you submit a signed request for termination or Form CMS-1763. We hope this helps.

    What is the time frame I have after getting married to change my last name?

      Hi Kristine. You should notify us of your name change, as soon as you decide to change it at work, with your bank, or any other institution. To change your name on your Social Security card, you must show us a document that proves your legal name change. If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records, you must show us an identity document in your “old” name (as shown in our records) and another document showing your “new” name.     A couple of months ago I completed most of my social security application on line. Since then my computer died and now I have a new computer. Info was transferred to this new computer however I cannot find my application. I do have the Social Security Application re-entry number and it is *******.
