Hello, my mother in law, Barbara N Regan passed away in 2019 on July 27th. I do her taxes. She lived at a senior living facility called the Renaissance, located at 20376 John Rd Apt 1022 Olmsted Twp, OH 44138-1276. The Renaissance has been forwarding her mail to my husband, Thomas, at our address. We have received other tax documents ( 1099’s etc.) but we have not received her SSA-1099. How can I get you to re-send it?

    Thank you,
    Molly Regan

The SSA-1099 is mailed to the last address in our records and is intended for the spouse or executor of the deceased. If you did not receive the benefit statement, the executor will need to go into the local Social Security office with proof of death and proof of executorship. An SSA-1099 can also be given to someone receiving benefits on the deceased beneficiary’s record such as a spouse, parent, or child of the deceased. If you need more information, call our toll-free number at 1-800-772-1213 (TTY 1-800-325-0778) and ask a representative for assistance. We hope this helps.
