New Option for Submitting your Disability Update Report!

We are required to conduct continuing eligibility reviews for disabled beneficiaries every three years. This process requires that beneficiaries complete a Continuing Disability Review mailer to update information about their medical conditions and recent treatments.

We now offer an online option to complete this update and provide any supporting documents about your medical treatment or your work.

We designed this new form with convenience in mind—and to save you time. You can access the online form. (Use either Microsoft Edge or Google Chrome for the best online experience.)

You will need your Social Security number, your current address and phone number, and a valid email address to complete the form. Also, you must have received a request for an updated disability report in the mail.

Once you “Click to Sign,” you will receive an email from echosign.com asking you to confirm your digital signature. Check your junk folder if you don’t receive it within a few minutes. Your signature isn’t complete—and your form won’t be processed—until you complete the instructions in your email.

    I currently reside at a nursing home facility and dont have access to my previous mailings, was there a specific date these reports were mailed out? If so, how can we verify we received a updated report?

    I filled out the lengthy continuing disability review report 6 months ago and mailed it in the provided envelope and I just received another one. I called the Lebanon, MO SSA office and a couple of ladies told me that they just draw names like the lottery. I just read that you send these out every 3 years. I am mentally & permanently physically disabled and my nerve damage in my back and 5 compressed vertebre, along with severe vitamin D deficiency makes this extremely hard and several pages repeat the same questions is literally too much to try to use my hands and my fingers go numb. I know the COVID situation has made a lot of offices get extremely overloaded and behind on the process of everything. I can’t believe I was just mailed another one of those Continuing reports in 6 months. My physical disability is worse every day and I have a broken ankle with pulled ligaments and tendons in my leg up to my shin because I face plant and my legs don’t work or move and I’m going to end up in a wheelchair. My backs permanently deformed and going to get better. Why in the world did i get another Report to fill out. Mentally and physically challenged is litterally pure torture and I don’t think I should have to do them when they are done every 3 years. Telling me that it’s a drawing like the lottery and pulled my name again is absolutely ridiculous and very unprofessional.
