It’s that time of year again. Preparing for tax season can seem overwhelming. Some forms and paperwork might be difficult to track down. If you misplaced your Benefits Statement or haven’t received it by the end of January, we’ve made it easy to go online to get a replacement with a personal my Social Security account.

I tried to apply on line for my SS replacement card.
two of the questions were about me opening a credit card in 2008, I don’t remember what card I would have opened 10 years ago. I have had many credit cards over the years.
Also, for a car loan, I have owned a number of automobiles and have had loans from different companies, I did not recognize the names you listed so I had to choose none of the above that I recognize.

I just want to know what information I need to bring with me when I go into the local Madison Wisconsin SS Office to apply for a replacement card ?

Hi Stephanie, if your step-father needs a replacement Form SSA-1099, he can get it by:
•Using his personal my Social Security account;
•Calling us at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 7 a.m. to 7 p.m.; or
•Contacting his local Social Security office.

I need a copy of my deceased father’s SSA-1099 for 2017. He passed away in March 2017. My mother is still living and is now receiving the monthly benefit that my father was receiving. She has received her SSA-1099 would this include my father’s benefit for January, February and March of 2017 or would there be a separate 1099 for those three months for him. I need this to file their 2017 taxes.
