New Option for Submitting your Disability Update Report!

We are required to conduct continuing eligibility reviews for disabled beneficiaries every three years. This process requires that beneficiaries complete a Continuing Disability Review mailer to update information about their medical conditions and recent treatments.

We now offer an online option to complete this update and provide any supporting documents about your medical treatment or your work.

We designed this new form with convenience in mind—and to save you time. You can access the online form. (Use either Microsoft Edge or Google Chrome for the best online experience.)

You will need your Social Security number, your current address and phone number, and a valid email address to complete the form. Also, you must have received a request for an updated disability report in the mail.

Once you “Click to Sign,” you will receive an email from echosign.com asking you to confirm your digital signature. Check your junk folder if you don’t receive it within a few minutes. Your signature isn’t complete—and your form won’t be processed—until you complete the instructions in your email.

    I have NOT received any letters to confirm my disabilities so far. But if I DO receive one, I would NOT do it online. With the thought of how often we’ve heard of hackers getting and breaching our personal/private information, I don’t do anything that includes any of my private information on the internet. I do it all by mail or in person.

    Your SSA online site access for my gmail & username has been data breached-blocked. Please have your Data Processing quality control personnel provide an alternative.

    I don’t know why I am getting these announcements. I do not get any SS benefits.

    I have not received a letter. It’s Nov 3rd (Election day), so I know mail has been overloaded with mailing ballots.
    But how do I know if I already have a letter in the mail or if there is still one coming?

    Function Report and Supplemental Seizure Report were completed and returned on 10/28/2020 and 10/26/2020 respectively by Representative Payee Brenda I. Klein, Mother and signed by Karen Marie Klein. Karen cannot read or write so her printed signature has been used for 51 years.

    No need for online report at this time since already completed and returned.

    I have received the letter about updating my disability status, but I’ve never gotten one before? I live alone in Ireland, but SS deposits my pmt to Chase Bank in US.
    Since 2003 I’ve been blind in my left eye (reported to IRS), and I have Type 2 diabetes, osteoarthritis (which required knee surgery in 2015) making mobility difficult. I also have a history of atrial fibrillation, requiring hospitalization in 2013. I have not been able to work since 2017.
    Am I missing out on increased benefits? Also, am I eligible for any payment supplements because of Covid-19; I received nothing over the past 9 months…?
    Thank you for any help you may be able to give me!
