Sign up for Medicare Part B Online, by Fax or Mail

For many people, signing up for Medicare Part B doesn’t require you to leave the comfort of home.  Please visit our Medicare Part B webpage if:

  1. You are already enrolled in Medicare Part A.
  2. You would like to enroll in Part B during the Special Enrollment Period.

You can complete form CMS-40B (Application for Enrollment in Medicare – Part B [Medical Insurance]) and CMS-L564 (Request for Employment Information) online.

You can also fax the CMS-40B and CMS-L564 to 1-833-914-2016; or return forms by mail to your local Social Security office.  Please contact Social Security at 1-800-772-1213 (TTY 1-800-325-0778) if you have any questions.  

 Note:  When completing the forms:

  • State, “I want Part B coverage to begin (MM/YY)” in the remarks section of the CMS-40B form or online application.
  • If your employer is unable to complete Section B, please complete that portion as best you can on behalf of your employer without your employer’s signature.
  • Submit one of the following types of secondary evidence by uploading it from a saved document on your computer:
    • Income tax returns that show health insurance premiums paid.
    • W-2s reflecting pre-tax medical contributions.
    • Pay stubs that reflect health insurance premium deductions.
    • Health insurance cards with a policy effective date.
    • Explanations of benefits paid by the GHP or LGHP.
    • Statements or receipts that reflect payment of health insurance premiums.

    I am in a similar situation. I applied for Medicare Part B on-line and printed the email that says my e-sig was complete. This was about 1 month ago. There seems to be no way to check status of application on the SS web-site. I called the SS office and the agent said he saw no evidence of the application. He advised me to submit it again via fax saying that “in his experience, that system works better than the on-line application”. He admitted there is really no way to check the status of my application.
    This is beyond frustrating. I am trying mail as I have no fax capability at home. So, agreeing with Dennis,– may not be wise to trust the on-line system. Fingers crossed the mail version works.

    I applied for Part B online via the Special Enrollment Period, completed the digital e-sign process and received an email indicating “You’re done signing Medicare Part B Online Application” and “After you have confirmed your electronic signature and we receive your application, we will:
    Review your application and contact you if we need more information or if we need additional documentation;
    Process your application once we have all of the necessary information and documents; and
    Mail you a decision letter.”. No confirmation number was provided. It has now been 47 days, and I have called SSA several times and they show no record of my application. I recommend that applicants DO NOT attempt to apply online, and instead submit their application by mail, which is what I am now going to do.

    I called the social security toll free number, after waiting for 2 hours,the rep refused to even look into my file to see whether or not they received the application. She said i just need to wait for 60 days. What if my online application never been received than what am i waiting for? I cant see the status online . The email that i received stated ou’re done signing
    Medicare Part B Online Application.
    It doesn’t not indicate that it was sent. I am not asking to expedite the process, all i wanted to know is whether or not it’s being received. Is this too difficult for ssi rep to do,is at least attempt to help a helpless citizen?

If you cannot check the status of your claim online using your my Social Security account, please call your local Social Security office. You’ll find the phone number using the Office Locator. Our call volume and wait times are longer than normal, so please be patient. We hope this helps.

    I registered for social security and i think i registered incorrectly. I still have a full time job and have insurance with the company i work for and they have over 20 employees. I just received a bill for Part B insurance and i don’t need any type of insurance at this moment. How do i disenroll for the program ?

    I have Part A and three weeks ago used this site to sign up online for Part B using the special enrollment period. I received an email from SS for me to do the digital signature, which shows a confirmation that it was signed. Just called Soc. Security Admin, and they have to record of it. They said it should be at my local office. That office has no record of it. Why is there no confirmation number provided when sending it online? What am I suppose to do?

        I think what readers should know about using the online Part B application is that Social Security uses an Adobe form which lets you fill in the application form, then Adobe emails you the filled out form for your digital signature. This then is automatically emailed to SS.gov. You are not filling out the form in the Social Security site, and you do NOT get a tracking number or any assurance that they received the document. So, even though I can prove that the email was sent to them, in my case they have no record of it. Honestly, this is a major failure of this system. Social Security should provide a tracking number to avoid this kind of situation.

      . After waiting for 2 hours,the rep didn’t even attempt to look into my file. She flat out saying i need to wait for 60 days. I don’t mind waiting, but what if my form has never been received?l then what am i waiting for? This experience is beyond disappointment

    what is the best fax number to use to submitt a CMS-L564 form. I submitted one covering the period jan.2020 thru december 2020, but was denied and must now submitt october 2018 thru jan.1st 2020, in order to prove there was less than an 8 month gap in health ins. I want to fax the form since my coverage on my wifes heath ins. plan changes when she retires at the end of this month
