New Option for Submitting your Disability Update Report!

We are required to conduct continuing eligibility reviews for disabled beneficiaries every three years. This process requires that beneficiaries complete a Continuing Disability Review mailer to update information about their medical conditions and recent treatments.

We now offer an online option to complete this update and provide any supporting documents about your medical treatment or your work.

We designed this new form with convenience in mind—and to save you time. You can access the online form. (Use either Microsoft Edge or Google Chrome for the best online experience.)

You will need your Social Security number, your current address and phone number, and a valid email address to complete the form. Also, you must have received a request for an updated disability report in the mail.

Once you “Click to Sign,” you will receive an email from echosign.com asking you to confirm your digital signature. Check your junk folder if you don’t receive it within a few minutes. Your signature isn’t complete—and your form won’t be processed—until you complete the instructions in your email.

    Why can Form SSA 455 be completed online but your administration has done nothing to accommodate people who need to submit Form 454 BK? Local offices are NOT processing anything. People are about to be cut off of their benefits because no one is scanning the mail coming in. Despicable.

    They now say I have cops and I have had more hard wear on ankle and getting ready to knee replacement what should I do are will help me more cause I’m under poverty way low

    I am confused about CDR. SSA gives you 30 days to complete or they cut you off. My case is under federal review, SSA asked for more then one extension citing difficulties related to Covid. Getting medical records is no easy task anymore. Many doctors, especially in psychology field no longer reply to SSA records request. I have been told, especially in PA that those records are under tight lock and key. Even with a release because they dont wont to provide incriminating information that can be shared. The medical field attitude has become SSA will just send to a 15 minute appointment of one of their doctors.

    Hello,

    Is there a backlog with the CDR for SSI? I’ve not yet received one but was supposed to in October. I naively thought they were temporarily suspended because of Covid.

    Also. Can one give the name and address of the doctor and have y’all call them? Or does one track down the paperwork and give it to y’all along with the CDR form?

    I live in Tennessee and it’s not the most modern state.

    Hi, I received a packet in the mail from social security about 2 weeks ago and filled it all out and sent it back. It was 15 pages front and back to fill out. I dont have to go online now to fill anything out.
    Do I???

        The local Social Security Offices are so bog down they are ignoring faxes and calls. The SSA Process need to be streamlined to assist the agency and the Participants

    OUR SON APPLIED FOR DISABILITY 3 TIMES AND WAS DECLINED, HIS LAST HEARING WAS FEB. 2015 AND THE JUDGE SAID SEE YOU IN 5 YEARS, WELL THE FOLLOWING YEAR HE PASSED AWAY WITH ALS. HE NEVER RECEIVED ANY ASSISTENCE AT ALL. BOTH HIS DAD AND I , WERE IN OUR LATE 70,S RETIRED BLUE COLLAR WORKERS ,AND WE HAD TO TAKE CARE OF HIM OURSELVES. OUR SON WORKED HARD ALL HIS LIFE UNTIL 43 AND THIS IS THE THANKS HE GOT. WHILE OTHERS SAY I AM DISABLED AND GET FULL BENEFITS ,INTERESTING… I SEE AND HEAR IT SO OFTEN AND THEN I THINK OF OUR SON AND WHAT HE GOT.WE JUST DIDN’T KNOW HOW TO WORK THE SYSTEM, YOU KNOW THAT HAPPENS AS WELL AS I .
