Businesses, banks, schools, private agencies, etc., are free to request someone’s number and use it for any purpose that does not violate a federal or state law.   1. We are a contractor working for the State which is interfacing with SSA to get the SSDI/SSB Income and also Medicare Information via the BENDEX process. We wanted to understand, how this change will impact the existing transfer mechanism with States. Our State derives the Medicare information from BENDEX.

  3. I have my new passport and I want a new social security card with my full name. I need to fill out a SS5 form. If I go down to the SS office what will I need and approximately how long will it take. Can I get the SS5 form online. I did not see the form.

  4. My Medicare card was stolen on 11/19/19…..I need to get it replaced ASAP, including a letter to cover the time in delivery.
    Please advise how to do this
